Updated October 2026. Orthopaedics is a small search market with very high prices. Across 24 deduplicated keywords, the volume-weighted top-of-page bid is S$30.49, ranking #1 of 15 in our Singapore Medical CPC Index and 4.7× the cost of a dental click. Even the low end of the bid range (S$9.41) is close to the high end in several other medical verticals. The priciest group this month is Shoulder, at S$38.80.

The figures come straight from the Google Ads API (Keyword Planner, Singapore, English) and are refreshed every month. Below: what each body area costs to advertise, the most expensive searches, seasonality, and what the HCS (Advertisement) Regulations mean for orthopaedic practices.

Orthopaedic CPC by body area

Keywords grouped by body area, with bids weighted by search volume. With so few keywords per group, a single term can move a group’s average sharply from month to month.

Treatment / service Keywords Searches / mo Top-of-page bid (low–high) Most-searched keyword
Shoulder 2 280 S$3.43–S$38.80 shoulder specialist singapore
Orthopaedic surgeon searches 7 2,690 S$10.92–S$32.04 orthopaedic surgery singapore
Knee & ACL 13 1,220 S$8.73–S$28.81 knee specialist singapore
Hand & wrist 2 300 S$4.19–S$15.68 hand surgeon singapore

Why orthopaedics is so expensive:

  • Surgical case value. An ACL reconstruction, shoulder repair or knee replacement in a private hospital is a five-figure case. Even after insurance, the margin on one surgical patient justifies paying S$37.23 for a “knee specialist singapore” click.
  • Insured patients with a choice. Many private orthopaedic patients pay through Integrated Shield plans or corporate insurance. They’re less price-sensitive, and when they are choosing a surgeon themselves, the surgeon who appears first has an edge. That pushes bids up.
  • A concentrated, specialist field. Private orthopaedic surgeons cluster around Mount Elizabeth, Gleneagles, Novena and the major private hospitals, and they all compete for the same few hundred monthly searches per term.

Two kinds of search lead the list. Pain-plus-specialist searches such as “shoulder pain specialist singapore” (S$45.28) and “knee pain specialist singapore” (S$31.30) come from patients who have decided they need a specialist but haven’t picked one. Procedure searches such as “acl surgery singapore” (S$43.03) come from patients who already have a diagnosis, often after a sports injury, and are choosing a surgeon.

The most expensive orthopaedic keywords in Singapore — Orthopaedic Google Ads CPC in Singapore: October 2026 Data

The two head terms, “orthopedist singapore” (1,000 searches a month) and “orthopaedic surgery singapore” (1,000), carry most of the volume, at S$32.06 and S$32.05. Knee replacement is cheaper per click: “knee replacement singapore” is S$23.62. Older patients are more likely to be referred, and much of that demand is likely subsidised.

Orthopaedic demand in Singapore is fairly stable, with dips around the festive period. Sports injury searches tend to follow the running and football calendars, and elective joint surgery is often timed around leave. In a market this small, a few dozen searches either way moves the monthly total, so treat individual months with caution.

Orthopaedic searches Google won’t show data for

We check the core orthopaedic terms by exact match every month:

Orthopaedic searches Google won't show data for — Orthopaedic Google Ads CPC in Singapore: October 2026 Data
Search term Keyword Planner, October 2026
orthopaedic surgeon singapore 260 searches, S$9.12–S$31.95

acl surgery singapore 70 searches, S$10.65–S$43.03
knee replacement singapore 110 searches, S$5.00–S$23.62

Unlike LASIK, IVF or mental health, orthopaedic terms are fully reported by Keyword Planner. You can see exactly how expensive the market is before you spend. The price is the constraint, not the data.

What patients search vs what your ads can say

Orthopaedic practices are HCSA licensees, so their advertising falls under the HCS (Advertisement) Regulations 2021 and the Singapore Medical Council’s ethical code. Patients search with the superlatives the rules ban:

These are the words patients type. Under the HCS (Advertisement) Regulations, a licensee’s ad copy cannot repeat most of them.
Searcher modifier Keywords Searches / mo Avg high bid Examples
“best” 4 280 S$23.86 best orthopaedic surgeon singapore, best hand surgeon in singapore, best knee specialist singapore
“top” 1 30 S$32.00 top orthopedic surgeons in singapore

For orthopaedic surgeons, the rules that matter most:

  • Laudatory claims. “Best knee specialist”, “top orthopaedic surgeon”, “highest volume surgeon”, “extensive experience” and “highly experienced” all appear on MOH’s list of terms likely to be treated as laudatory.
  • Unjustified expectations. “Back to running in 6 weeks” or “pain-free after one injection” promise outcomes and timelines that not every patient will get.
  • No comparisons with other surgeons or clinics, even unnamed ones.
  • Testimonials. Organic reviews posted directly by patients are fine. Reproducing athlete or patient testimonials in ads, or paying for them, is not.

MOH’s FAQ states that SEO “is not considered as ‘advertising’”, while paid search is “governed by the HCS (Advertisement) Regulations”. The content still has to comply, but rankings are earned through relevance, authority and patient reviews, not through laudatory copy.

Ads vs SEO for an orthopaedic practice

If one practice ranked first organically for every keyword in this dataset, the clicks would be worth roughly S$22,393 a month in equivalent Google Ads spend, from only 3,740 searches in September 2026. Orthopaedics has one of the strongest cases for SEO in the index: each organic click replaces a S$9.41–S$30.49 paid one.

Ads vs SEO for an orthopaedic practice — Orthopaedic Google Ads CPC in Singapore: October 2026 Data

A practical split:

  • Google Ads on a few procedure terms you most want, with tight geography and schedules, factual copy, and landing pages that explain the procedure and list fees.
  • SEO for condition and symptom content (knee pain, rotator cuff, slipped disc), procedure explainers and surgeon profile pages that build the authority Google looks for in medical results.

The same logic applied in our Harley Street Heart & Vascular Centre engagement, a specialist practice in another high-CPC field. All five commercial target keywords reached page one within eight months.

Methodology

How this index is built. Every month we pull Google Ads Keyword Planner data through the Google Ads API for Singapore (English, Google Search network). We seed each medical vertical with its core treatment terms, then:

  • Collapse close variants. Keyword Planner reports “boob job singapore” and “breast implant singapore” with identical numbers. We count them once.
  • Drop navigational searches. Clinic and hospital brand names are excluded, along with anything under 30 searches a month.
  • Report the top-of-page bid range (low and high) in SGD. Our headline “CPC” is the volume-weighted high end: what it typically takes to show above the organic results.
  • Probe restricted terms by exact match to record where Google withholds data or shows no bids.

Top-of-page bids are Google’s forecasts, not any one clinic’s actual cost per click. Quality Score, ad schedule, device and competition on the day move real CPCs above or below this range. Use the index to compare treatments and verticals, not to set a single bid.

High surgical case values, insured patients, and a concentrated group of private surgeons bidding on a small number of searches. Orthopaedics ranks #1 of 15 verticals in our index.

Can orthopaedic surgeons advertise their experience?

Factual qualifications and registered specialist titles are fine. Phrases such as “extensive experience”, “highly experienced” or “highest volume surgeon” appear on MOH’s list of likely-laudatory terms and should be kept out of ads.

Is SEO or Google Ads better for an orthopaedic practice?

Because clicks are so expensive, SEO usually gives a better long-term return, while ads fill gaps for specific procedures. See also physiotherapy CPCs and specialist clinic CPCs.
